Hampton Bay Torchiere brushed nickel floor lamp. Seventy-two-inches high with a thirteen-inch diameter base. The shade is glass with a frosted alabaster finish. I do not know if you can still buy these lamps with a glass shade - they all seem to be plastic. The shaft unscrews into three sections - easy to dismantle and move. This lamp works but has one odd problem - the concrete weight in the base has disintegrated. Otherwise the lamp is in perfect condition having sat in the corner of my smoke, kid, and pet free bedroom for years. If you have a large disc of cast iron or steel that would fit in the base you could fix this lamp. I have the original box and instructions. Three way switch for an incandescent bulb, but I found it works fine with the switch in the 100 watt position with a LED bulb operated from a wall switch.
