Primary Position Duties:
1.Assists client to live in a clean, safe environment. This may include: cleaning bathrooms, and kitchen, vacuum and dusting, scrubbing floors, changing bed linens, cleaning refrigerator, laundry and personal errands.
2.Plans and prepares nutritious meals including modified diets as assigned. Encourages client to maintain proper nutrition and maintain adequate fluid intake.
3.Provides companionship and socialization to clients in their home.
A.Assists client in arranging medical appointments and completing necessary shopping and errands as assigned. Maintains written receipts and records for shopping and errands.

4.Supports the client's independence by:
A.Reminding client to take prescribed medications that have been set up by the nurse.
B.Reminds client to perform exercises as established in the plan of care.

5.Follows infection control policies including proper hand washing technique.
6.Documents accurate and complete record of observations and duties performed.
A.Reports significant changes in client's condition to the RN Case Manager immediately.
B.Participates in the client's care conference, as requested.

7.Maintains absolute confidentiality of all information pertaining to clients, families and employees. Accepts and fulfills assignments with the Company and exercises judgement in accepting assignments.
8.Performs other related duties and responsibilities as assigned by the Branch Office.

Education or Skills:
Heavy to Very Heavy physical demands. Involves bending, stooping, lifting and stretching. Must be adaptable to a variety of environments and settings in the community. Must be able to drive or use public transportation.

Exerting in excess of 100 pounds of force occasionally, in excess of 50 pounds of force frequently, in excess of 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for heavy work.

Position Requirements:
1.Be at least sixteen (16) years of age.
2.Ability to read, write, follow written plan of care and document tasks performed.
3.Self-directed and work with minimal supervision.
4.Demonstrate dependability, tact, patience and the ability to follow instructions.
5.Strong interpersonal communication skills.
6.Have experience in cooking, cleaning, laundry and shopping.
7.Proof of negative mantoux or documentation of negative chest x-ray.
8.Access to a dependable vehicle or public transportation in order to travel to multiple company business stops a day.
9.A valid driver’s license and proof of car insurance when using a personal vehicle for company business.
10.Have U.S. Citizenship or evidence of valid Alien Work Permit.
11.Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services. A candidate who has been convicted of such crimes will be hired.
12.Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.
