Primary Functions of the Position
Under the direction of the Youth and Family Services Program Director, the Mentor position is responsible for planning and implementing mentoring services to Hmong adolescents in the juvenile justice system and to lead a variety of program activities for program participants.
The YouthLEAD Coordinator develops and coordinates YouthLEAD services intended to help youth set and achieve education goals, learn about career opportunities and develop employment skills. The YouthLEAD Coordinator develops activities and/or identifies resources that will engage and motivate youth in planning for their future. He/she will recruit 20 to 30 youth annually and provide case management services including initial assessment, developing individual education/employment goals and monitoring progress. The YouthLEAD Coordinator provides all data validation and reports required by the Minnesota Department of Employment and Economic Security and works effectively with HAPs project partner, Guadalupe Alternative Program (GAP).
a) Major Position Responsibilitiesfor the Mentor position:
1) Programs/Services Achieve Results: develops, implements and manages the programs that consistently achieve prescribed and/or pre-determined outcomes.
Provides overall case management for 20 -30 high risk Hmong adolescents.
Supports Hmong families who struggle with the intergenerational and bi-cultural struggle of Hmong American adolescence.
Assists Hmong teenagers in building a positive, bi-cultural vision of their future. Assists Hmong teenagers and their families in maximizing educational opportunities.
Coordinates all program activities with HAP, community agencies and schools to ensure that participants receive program services in a respectful, efficient, accurate and timely manner.
Promotes a positive working relationship between the Peem Tsheej program, community and Hmong American Partnership personnel.
Provides intake and needs assessment of 20 - 30 teenagers annually.
Provides case management services to Hmong teenagers and their families including assessment and developing individualized case plans. Monitor progress on goals through regular individual meetings and home visits.
Provides one-on-one mentoring and family circles. Provide support as needed to program participants and their families on educational concerns, job searches, housing, and family conflicts.
Coordinates all details of weekly Aftercare support group and activities and case planning and management for enrolled youth.
Works cooperatively with Peem Tsheej team to organize family gatherings, family mediation and other family activities.
Contacts Peem Tsheej program participants parents for program orientation and to obtain written permission for the teens to participate in the program. Obtains individual activity permission slips for off-site field trips.
b) Major Position Responsibilities for the YouthLead position:
1) Programs/Services Achieve Results: develops, implements and manages the programs that consistently achieve prescribed and/or pre-determined outcomes.
Develops a comprehensive inventory of activities and identify available community/school resources that help youth become aware of access to education opportunities, financial aid, career opportunities and job skills.
Creates on-the-job-training and employment resources with community businesses for participating youth.
Organizes and schedule an annual calendar of events/activities to engage youth and achieve goals.
Recruits approximately 30 to 40 youth annually to maintain program enrollment at 30 participants throughout the year.
Provides case management services to each participant, including initial assessment and registration, development of education and employment goals and monitor and record progress.
Leads program activity, recruit volunteer presenters, refer participants to community/school resources.
Collects and maintains data, prepares and submits all reports required by the Minnesota Department of Employment and Economic Security.
Works effectively with project partner (GAP), including initiate and maintain communication regarding program activity, issues and results; attends County YouthLEAD meetings and submit reports as requested.
2) Revenue and Expenses are effectively managed in concert with the Program Director:
Develops and manages the program budgets so that revenues meet expenses.
Manages budget/spending for Peem Tsheej programs as needed to ensure program goals are met.
Closely monitors all funded programs and services to insure funds are expended according to approved budgets and that disbursements are made in a timely fashion.
3) Results are accurately Documented and Reported by Deadline:
Develops, implements, and fine-tunes administrative systems and processes that establish clear service standards and facilitate reporting.
Maintains accurate records and reports of Peem Tsheej program activities.
Prepares monthly reports of participation to be submitted to the Program Director on last day of each month.
Gathers data from the Peem Tsheej program to support the Youth and Family Department quarterly report.
Creates and maintains a client file for each program participant.
Ensures demographic information about overall activities is collected and maintained to provide to funders, and to advise the ongoing and future work of HAP.
Provides other reports on activities and projects as requested.
4) Strong Team Player: Contributes to the overall effectiveness of HAP in fulfilling its mission and objectives
Maintains strong working relationships with juvenile justice service systems, specifically (but not limited to) Boys Totem Town staff; the Ramsey County Director of Juvenile Services, the Supervisor of the Ramsey County Northend Probation Office, and probation officers working with enrolled youth.
Represents Peem Tsheej program to parents, schools, funding sources, and the community.
Attends meetings as needed.
Represents and promotes the agencys mission and programs in a variety of public settings.
Serves or attends outreach recruitment community events.
5) Customer Service: Role models a customer oriented service philosophy at all times.
Models customer service to all clients/customers by showing courtesy, respect, honesty, confidentiality, and professionalism.
Knows and follows the HAP Customer Service Policy.
6) Adherence to HAP policies:
Knows and follows the HAP Employee handbook.
Ensures compliance with all contract regulations and government laws.

Skills, Knowledge and Abilities
Ability to manage complicated partnerships and management agreements
Ability to manage and track multiple projects simultaneously
Ability to work with diverse staff and organizations, and provide effective supervision to a multi-lingual and multi-ethnic staff
Ability to work occasional evenings and weekends as needed
Effective leadership style that promotes positive customer services that effectively connects the participant with the needed services
Effective bi-lingual communication skills both written and oral in English and at least one other language
Effective group facilitation and meeting management skills
Ability to work independently with minimal supervision, while also being a strong team player
Knowledge and experience in program development and program evaluation
Ability to read, interpret and effectively manage the department budget
Familiarity and comfort with electronic forms of communication, including ready access and responsiveness to e-mail correspondence
Knowledge of the barriers both staff and participants confront on a daily basis and abilities in finding solutions to these barriers
Ability to interpret legislation and regulations, and effectively integrate and communicate these rules and regulations with programs and staff.
