Metropolitan Health Plan (MHP) is now seeking a Care Coordination Manager to supervise case managers who evaluate and enhance the care of MHP members, and ensure that all medical and regulatory standards are met in accordance with federal, state and accreditation guidelines. MHP offices are located in the Grain Exchange Building at 400 South Fourth Street in downtown Minneapolis.

Primary duties and responsibilities include:
Supervise Medical Administration staff who provide case management for Maternal and Child Health, Complex Care, Senior and Adult Care, Disease Management and Clinical Data Performance; ensure that staff provide consistent, member-focused care.
Develop and maintain case management policies and procedures.
Assess, re-design and coordinate case management processes.
Integrate medical and behavioral health care processes for MHP members.
Ensure that staff conduct pre-service, concurrent and retrospective case reviews within prescribed timeframes.
Monitor levels of care and make recommendations for interventions.
Interact and coordinate with medical directors to ensure practice guidelines are monitored and integrated into member care plans.
Ensure that continuity of care (including utilization management, transfer, discharge and referral to outside agencies and providers) is managed by case managers.
Assist staff in management of complex cases.
Work with ancillary departments to ensure all processes are aligned.
Report updated utilization and regulatory data to senior management.
Identify success criteria and measure effectiveness.
Ensure that member privacy and confidentiality are maintained during daily operations and activities.
The Ideal Candidate will have:
One of the following:
Master's degree in public health, public administration, nursing, social work or other related field plus three or more years of professional work experience in a health plan related setting.
Bachelor's degree in public health, public administration, nursing, social work or other related field plus four or more years of professional work experience in a health plan related setting.
Experience working with government health products (e.g., MSHO, MSC+, PMAP, Minnesota Care, SNBC and Medicare/Medicaid).
Experience supervising staff in a health related setting, and managing personnel matters that include interviewing, hiring, evaluating, disciplining and terminating employees.
Knowledge of federal, state and regulatory health plan compliance requirements.
Experience integrating quality components into daily operations to meet administrative and regulatory standards.
Experience managing cross-funtional projects.
Experience analyzing utilization management data and creating appropriate interventions.
Experience creating a case management model applicable to culturally diverse Medicare and Medicaid populations.
Experience developing and providing education for clients and staff.
Experience using Microsoft Office, including Word and Excel.
Ability to communicate effectively both orally and in writing, and to establish good working relationships with the general public, staff and other health professionals.
Ability to organize and coordinate program activities.
Ability to demonstrate the Hennepin County Supervisor Competencies.
